Crash when using BinMD dialog

Description

On Windows if you try to open the BinMD dialog you can get a crash.

Re #5911. Fixed the name clashes of UI classes created with designer.

The reason for the crash is well understood.

Was working with BinMD interface for a month -- no more crashes occurs.

The dialogue creator generating the same dialogue names for different custom dialogues (what causes this crash) should be fixed by another ticket or at least well informed to developers, who is creating custom dialogues.
