Toolbar placement needs rethinking

Description

At the moment the toolbars in MantidPlot are a little confusing. They appear when you click a plot but do not disappear when focus is removed. They also tend to get rearranged and this can lead to confusion.

Also most importantly the starting location for the Graph zoom toolbar is almost hidden. These need better starting positions.
